We’re always happy to welcome new pets (and their people!) to our practice. Registering with Medicor is quick and easy — just fill out the form below, and we’ll take care of the rest.

Once you’ve submitted your details, you’ll receive an email with a copy of your registration form for your records. We’ll be in touch shortly to confirm that your pet has been successfully registered.

If your pet has been seen at another practice, we can also help you transfer their medical history — just let us know in the form or give us a call.
